REGISTRATION

To enroll your child (children) in Schoolzin Schoolzout, you must first complete and submit the following forms:

-	Registration Form
-	Signed Parent/Guardian Center Agreement
-	Emergency Medical Authorization
-	Vehicle Emergency Medical Authorization
-	Current Georgia Immunization form 3231
-	Infant feeding plan (if applicable)
-	Income eligibility statement for the Georgia Food Program

These forms will be provided to the parent/guardian along with a parent handbook on what to expect and the services we provide.


Tuition

If you receive state child care assistance, you must pay your parent fee set by the state. A discount of 5% is given for those parents with 2 or more children enrolled full time, as well as a 10% discount for tuition paid a month in advance. Parents who receive state child care assistance are not eligible for the 10% monthly discount.
